Many hospitals that oversee a large volume of patients have high readmission rates -patients falling ill after leaving the hospital- for numerous reasons including a lack of a concrete remote monitoring system and patients not complying with their post-discharge plan.  Lowering readmission rates optimizes hospital resources, helps avoid possible fines from Medicare, and improves overall patient health outcomes. Reduced re-admissions are necessary to lower overall costs, improving the healthcare system for both patients and hospitals. 

Our product is a system composed of both patient-side and doctor-side applications. The patient side allows patients to better understand and manage their post-discharge plan, including a medication schedule, necessary doctor recommendations, artificial intelligence suggestions based on patient history, and checking for symptoms. Doctors can monitor changes in patient health and symptoms to update their post-discharge care accordingly and answer questions.
